All clinical and medical records are stored exclusively in CharmHealth, our HIPAA-compliant Electronic Health Record (EHR) system. We do not store or transmit protected health information (PHI) via email, spreadsheets, cloud storage, or local devices.

We are legally required to:

  • Maintain the privacy and security of your protected health information

  • Inform you promptly if a breach occurs that may have compromised your information

  • Follow the terms of this notice and provide you with a copy

  • Not use or share your information in ways not described in this notice without your written authorisation
